If you are planning to do more than one workshop, please hit the "Back" button at the top of your browser. You will be returned to the form without erasing the material you have entered. If you find you have made a major error in your information, please press the "Back" button, correct the information (making sure to indicate that this is a "REPLACEMENT" form) and resubmit. If you have multiple similar forms, I'll use the last form which was entered as the correct one.
